Patients: Please download a copy of the request form and take it with you to the next appointment with your treating practitioner. Please discuss your eligibility with your treating practitioner. Please note: In order for the collection sample to be viable on receipt in Germany, timing and transport of the sample is critical. Blood collection can happen on a Monday only.
For patients living within the major CBD areas (Sydney, Melbourne, Brisbane, Adelaide, Canberra, Perth, Hobart, Auckland, Christchurch and Wellington), the sample can be picked up by our express DHL courier.
Patients living outside of Sydney, Melbourne, Brisbane, Adelaide, Canberra, Perth, Hobart, Auckland, Christchurch and Wellington CBDs will need to transport the test to certain courier depots themselves. Depending on place of residence, this may involve a fair deal (hours) of driving so please be prepared for this.

2. TEST REQUEST
i) Complete form: To order a test, please download and complete a copy of the test request form above. The test must first be authorised by a certified, registered health practitioner and associated medical history information provided. Please read the form carefully and fill in both pages of the form. Patients are to fill in the yellow sections of the form and practitioners are to fill in rest of the form.
ii) Signatures: Please ensure both practitioner and patients have signed the request form in the appropriate places. Genostics will not proceed with the test unless these signatures are present. This is a legality and is strictly adhered to.
iii) Send the completed test request form to Genostics to complete the order. The form can either be faxed (+61 2 8088 7097) or scanned and emailed (firstname.lastname@example.org). Test requests will only proceed if signatures and payment details are completed.
3. THE TEST KIT WILL BE SENT TO THE PATIENT
Genostics will send the test-collection kit to the patient's address as written on the request form. Test kits are sent the same day as requested. If if an alternate address is desired please confirm this on the day the request form is sent, by calling our office on 1300 282 482.

4. COLLECTION AND DISPATCH
Practitioners and patients must discuss where the blood collection takes place. This may be at the practitioner's clinic or at a local pathology centre. Local pathology centres will charge a fee for this service.
Requirements (all patients):
Blood collection and courier pick-up must take place on a Monday only
Test collection kit contains 2 x EDTA tubes required for the test.
For patients residing WITHIN the CBDs of Sydney, Melbourne, Brisbane, Adelaide, Canberra, Perth, Hobart, Auckland, Christchurch and Wellington:
Book the courier early in the morning on the day of blood collection - MONDAY ONLY
For patients living outside of Sydney, Melbourne, Brisbane, Adelaide, Canberry, Perth, Hobart, Auckland, Christchurch and Wellington CBDs:
The completed test kit will need to be dropped of at a courier depot by the patient. Please note the Courier Depot location nearest you as well as the cut-off times for sample drop-off; this information is provided in your Test Collection Instructions contained in your kit. The nearest depot may be many hours from your place of residence so please ensure you have pre-planned the sample-drop off before proceeding with the test.

Even if the test is collected in a location that is normally accessible for the courier, the test will still need to be personally dropped off at a depot. Unfortunately the courier can only guarantee timely delivery of the test to Germany if the test is collected in the aforementioned CBDs or personally dropped-off to specific depots. Please do not organise a personal courier to deliver the blood to these depots: this adds an additional day of transport meaning the sample will not reach Germany by the Friday and thus be spoiled.
Once the kit has been sent, please fax through the fax-back form with details of the courier consignment, so that the laboratory can expect the arrival of the test. The fax back form will be in the kit provided.
Test results will be sent to the requesting practitioner within 12-14 days. Please note the Tumour Sphere Units test requires growth culture and results may take 4-6 weeks.
