HOW TO ORDER

Ordering Procedures and Protocols:

In order to proceed with an exacta® analysis, Genostics require the following:

1. Request Form and Consent to Obtain Biopsy Form (if required).

   • Form needs to completed, signed and dated by the requesting Practitioner and patient.

2. Copy of the histopathology report.

Once Genostics has received the above documents, we will retrieve the biopsy from the relevant pathology lab. Following receipt of the solid sample, we will then express ship the sample to the laboratory in the United Kingdom and India for further analysis.
