How to order

1. PREPARATION

Patients: Download a copy of the request form below and take it with you to the next appointment with your treating practitioner. Suitability for testing must be discussed with your treating practitioner. Please read the information below regarding how to proceed with a Maintrac test. NOTE: In order for the blood sample to be viable upon delivery to the laboratory in Germany, timing and transport of the sample is critical; blood collection must occur on a Monday only.

For patients residing WITHIN the CBD of Sydney, Melbourne, Brisbane, Perth, Adelaide, Canberra and Hobart, the sample can be picked up by a DHL courier.

For patients residing OUTSIDE the CBD of Sydney, Melbourne, Brisbane, Perth, Adelaide, Canberra and Hobart will need to drop the test collection kit off at a nominated FedEx depot. Depending on your place of residence, this may involve several hours of driving so please be prepared for this.

2. TEST REQUEST FORM

i) Complete form: To order a test, please download and complete a copy of the test request form above.  The test must first be authorised by a certified, registered health practitioner and associated medical history information provided. Please read the form carefully and fill in both pages of the form as well as the Medical Questionnaire.

ii) Signatures: Please ensure both the practitioner and patient have signed the request form in the appropriate places. Genostics will not proceed with the test unless these signatures are present; this is a legality and is strictly adhered to.

iii) Send the completed test request form to Genostics to complete the order. The form can either be faxed (+61 2 8088 7097) or scanned and emailed (admin@genostics.com.au). Test requests will only be processed if signatures and payment details have been completed.


3. THE TEST KIT WILL BE SENT TO THE PATIENT

Genostics will send the test-collection kit to the patient's address as provided on the request form. Test kits are sent the same day that a request form is submitted, so long as all of the details have been completed in full. If an alternate delivery address is required, please confirm this on the day the request form is submitted to us, by calling our office on +61 1300 282 482. 

Please read the instructions provided in the Test Collection Kit thoroughly and carefully. These instructions contain important information regarding preparation, collection and shipment.

4. COLLECTION AND DISPATCH

Practitioners and patients must discuss where blood collection will take place. This could be at the practitioner's clinic or at a local pathology centre. Local pathology centres may charge a fee for this service.

Requirements (all patients):

Blood collection and courier pick-up or drop-off must take place on a Monday only.

Test collection kit contains 2 x EDTA tubes required for the test.

For patients residing WITHIN the CBD of Sydney, Melbourne, Brisbane, Perth, Adelaide, Canberra, and Hobart: Arrange for blood collection Monday morning. Return to your pick-up location and call FedEx PRIOR to 10:00am to book a pick-up.

For patients residing OUTSIDE the CBD of Sydney, Melbourne, Brisbane, Perth, Adelaide, Canberra, and Hobart: the test collection kit will need to be dropped off at a designated FedEx depot. Please note the depot location nearest you as well as the cut-off times for sample drop-off; this information is provided in the Test Collection Instructions provided with your kit. The nearest depot may be several hours from your place of residence so please ensure you have pre-planned the drop off PRIOR to proceeding with the test.

Even if the test is collected in a location that is normally accessible by couriers, the test may still need to be personally dropped off at a nominated FedEx depot. Unfortunately the courier can only guarantee timely delivery of the sample to Germany if the shipment is picked up in the aforementioned CBDs or personally dropped-off to one of the specified FedEx depots. Please do not organise a personal courier to deliver the blood to these depots. Doing so adds an additional day of transport meaning the sample will not reach Germany within the allotted time frame and will no longer be viable for testing.

Following courier pick-up or drop-off, you must either fax the Fax Back Form to +61 2 8088 7097 or scan/email it admin@genostics.com.au. Once we have received the Fax Back Form, we can then notify the laboratory of your incoming sample. The Fax Back Form is provided with your Test Collection Instructions inside your kit.


5. RESULTS

The turn around time for results is approximately 10 days. Results are released directly to the requesting practitioner. Note: Tumour Sphere Units requires growth culture and results for this test can take approximately 4 weeks.
